MP is an 85-year-old female residing in a long term care facility. She was recently found on the floor near her bathroom by an aide doing nightly rounds. A nurse was called to assess the situation. Due to noticeable confusion and weakness MP was sent to the hospital for further evaluation.

While at the hospital MP was diagnosed with dehydration and a urinary tract infection. She was sent home shortly after with recommendations to increase fluid intake, antibiotics and to follow up with her PCP in one week. This was MP’s 4thUTI over a 6 month period of time.

Back at the long term care facility MP’s usual aide noticed she was not eating or drinking well. A dietitian was consulted to provide recommendations for reoccurring UTI’s and dehydration.
