Submit the completed copy of your note-taking sheet here.
If needed convert the note-taking sheet into a pdf and then submit the pdf format.
1. Click, "File."
2. Hover your cursor (the arrow) over "Download".
3. Select, "PDF Document".
4. In the Formative, click "Upload File Response."
5. Click "Upload File."
6. Find your Note-Taking Sheet in your file folder and click on it.
